Hudson City School District offers Life/AD&D and Voluntary Life coverage through Mutual of Omaha.
Life Insurance
Your amount of life insurance is an amount equal to 1 times your Annual Earnings, but in no event less than $10,000 or more than $150,000. Your amount of life insurance will be rounded to the next higher multiple of $1,000.
Accidental Death and Dismemberment (AD&D) Insurance
Your amount of accidental death and dismemberment (AD&D) insurance is equal to your amount of life insurance.
Benefit Reduction
As you grow older, the amount of Life and AD&D insurance for you will be reduced according to the following schedule:
Supplemental Life Insurance
Employee
- You may elect to be insured for an amount of life insurance from $10,000 to $200,000, in increments of $10,000. In no event shall your amount of life insurance exceed 5 times your annual earnings, rounded to the next higher multiple of $10,000.
Spouse
- You may elect to have your spouse insured for an amount of life insurance from $5,000 to $100,000, in increments of $5,000, provided the amount elected does not exceed 100% of your amount of life insurance.
Dependents
- You may elect to have your eligible dependent child(ren) insured for an amount of life insurance equal to $10,000, provided the amount elected does not exceed 100% of your amount of life insurance. Each eligible dependent child must have the same amount of insurance.
Voluntary Accidental Death and Dismemberment (AD&D) Insurance
Employee
- Provided you have elected some amount of life insurance, your amount of accidental death and dismemberment (AD&D) insurance is equal to your amount of life insurance.
Spouse
- Provided you have elected some sort of life insurance for your spouse, your spouse’s amount of accidental death and dismemberment (AD&D) insurance is equal to your spouse’s amount of life insurance.
